There are several advantages to dronedarone.

Unlike dofetilide and sotalol, there is a low risk of ventricular arrhythmias, it does not require hospitalization, and it is not renally excreted and can be used in the setting of renal insufficiency.

🔸Similarities Between Amiodarone & Dronedarone:

Amiodarone and dronedarone have similar molecular structures, but because of the molecular differences and absence of iodine, dronedarone is free of most of amiodarone's organ toxicity with only rare cases of liver toxicity. FDA instructions indicate that liver function tests should be repeated after initiation of dronedarone.

Dronedarone has a number of side effects.
🔹Nausea and diarrhea are the most common.

1. Dronedarone elevates the serum level of digoxin; a general rule of thumb is to halve the dose of digoxin given and to follow digoxin serum levels.

2. Dronedarone should not be used with Class I or other Class III drugs

3. Patients on dronedarone should avoid grapefruit juice

4. Observe for AV conduction abnormalities if the patient is also on beta-blockers or calcium channel.
