Sotalol- Pharmacology

acts on potassium channels and causes a delay in repolarization of the ventricles. It also has beta-blocking activity.

🔸Similarities Between Sotalol & Dofetilide:

Both sotalol and dofetilide cause prolongation of the QT interval and potentially cause life-threatening ventricular arrhythmias called torsades des pointes.
FDA regulations indicate that both medications should be administered to patients in the hospital using ECG telemetry initially for 5 doses and ECGs should be obtained 2-3 hours after each of these doses with strict instructions for discontinuation if the QT is prolonged beyond a specific value.
Both medications should be started only in patients with normal baseline QT intervals and not on drugs that prolong the QT.
The dosage should be adjusted based on creatinine clearance (CrCl). Sotalol should not be used in patients with CrCl less than 40 mL/min. Dofetilide should not be used in patients with CrCl less than 20 mL/min.



🔹Similar Side Effects:

Both drugs may cause life-threatening ventricular arrhythmias. Because both are renally excreted, the Cr should be followed so that adjustments in dosage can be made.
Dofetilide has minimal effects on heart rate and blood pressure while sotalol may cause significant heart rate slowing.
